Blaming The Individual When It Is Really The Organisations Fault

Ever had someone tell you something that you knew was wrong but we’re forced to comply with their instruction because of their position of influence?

Ever seen a member of staff blamed for doing a technique wrong (not exactly as it is shown in the manual) when the real problem is that the technique was difficult to learn in the first place?
Ever known anyone have a set of rules enforced upon them when they know they are unworkable, but are made to adopt them all the same?
If you have then you have just witnessed an identifiable failure under a Health & Safety Model called HSG48, which is a model I have used many times in cases I have been involved with to highlight how certain people in positions of influence make up rules to cover their inadequacies or because they are being relied on too heavily, without the necessary training and support, because they are expected to know because of their former job.
